PAYMENT POLICIES

Medical Weight Loss Center

Obstetrics & Gynecology
in Crystal River, Florida must collect all fees
and/or co-payments your insurance may require at the
time our Gynecological services are administered (we
do not accept insurance on Weight Loss Center fees.)
We will gladly file insurance with those plans with
whom we participate. We accept local checks, debit
cards, major credit cards and even cash.
If we do not participate with your insurance plan,
you must make full payment for all services rendered
at the time of your visit. We will gladly help you
file any necessary paperwork, and your insurance
carrier will reimburse you according
to its practices and policies. Returned checks will
be subject to the maximum fees the law allows. |

can help you achieve your healthy living and fitness
goals.
We
celebrate the diversity of our patients and
welcome all women regardless of race, religion,
sexual orientation, national origin, or marital
status.
We see patients by appointment from 8 a.m. through
12 p.m. and 1 p.m. through 5 p.m. Monday through
Friday and we are open late Tuesday until 6 p.m. We
will make every effort to schedule your appointment
at the most convenient time for you. If you cannot
keep your appointment, we understand, but please let
us know of your cancellation at the earliest you
possibly can so we may offer that time to someone
else.
Please bring your insurance card(s) and a photo ID
to every visit. If you take any medicines on a
regular basis, please bring them as well.

STATEMENT
The "Health
Insurance Portability and Accountability Act," also
known as "HIPAA," requires we inform all patients of
our privacy policies with a "Notice of Privacy
Practices" to explain how your personal information
may be used and disclosed, and how you can
access your individually identifiable health
